Elite Program

This program is an all-inclusive health service that includes unlimited care and support from your inter-professional team based on your personal health plan.

Your initial health assessment uses the most progressive techniques and equipment available, and it gives your professional team a comprehensive “snapshot” of your current health. From there, your health will be monitored and supported by your personal health team throughout your life. You will never have to remember when to come in for a routine test or examination again – or worry about whether you’re doing all of the things you’re supposed to. We’ll call you. And you get all of the time and attention you deserve.

Risk Assessment and Health Planning Stages

Every person in the member program receives the following services:

Stage 1: Pre-Assignment
Based on your initial intake and medical questionnaire, you are pre-assigned a team of professionals that represents the best match between your disease risks (or afflictions) and the specific expertise of the professional team.

Stage 2: Risk Assessment

You complete a comprehensive medical and family history questionnaire that assesses your relative risk of disease for multiple cancers and chronic diseases.

Stage 3: Physician Intake

The physician that is assigned to you conducts a thorough prevention screening consultation. They determine the precise compliment of screening tests and examinations that should be conducted, validate the inter-professional team assignment and begin creation of your personal health plan.

Stage 4: Screening Lab

Based on the physician’s recommendations, you undergo a series of examinations and tests that range from relatively simple blood tests to sophisticated imaging technologies. If the health team recommends repeated tests, they will be conducted on a prescribed schedule as outlined in your health plan.

Stage 5: Team Assessments

You receive an in-depth assessment by a kinesiologist (exercise specialist) and a registered dietitian to establish a fitness and nutrition profile. Upon the recommendation of either the kinesiologist or the supervising physician, you are also entitled to a physiotherapist assessment to establish functional impediments to sustainable fitness activity. Upon the recommendation of the physician, an on-site psychological health assessment may also be arranged.

Stage 6: Health Plan

The inter-professional health team meet as a group to formulate a comprehensive health plan for you in 4 specific areas (clinical/diagnostic, nutrition, body function & fitness and stress/mental health). The team prescribes routine tests/examinations, counseling sessions and coaching interventions as required. They also establish measurable health goals that can be monitored over time to show you the benefits of adhering to your health plan.

Stage 7: On-going Support

You are assigned a personal lifestyle coach that is cross-trained in medicine, nutrition, fitness and counseling - but whose core training is in a field most relevant to your profile. The coach makes regular contact with you to provide unrestricted support and assistance under your health plan. They will also coordinate additional consultations with the rest of the multi-disciplinary team as outlined in your plan.

Although your health plan will be continually refined according to your progress, each year this multi-stage program is repeated to ensure that we are applying the best resources to ensure you achieve your health objectives.
